Licensed Clinical Coordinator Home Health

Empath Home Health , a member of Empath Health , is seeking a Home Health Clinical Coordinator . The Home Health Clinical Coordinator is responsible for ensuring a smooth transition from hospital to home by coordinating with facility staff, patients, and families to facilitate timely and appropriate admissions. The Home Health Clinical Coordinator will work in the Naples, FL area and possess a valid clinical FL license.

Location: Naples, FL (Empath Naples office)

• Meet with patients and families at the bedside to introduce home health services and provide education on care
• Review and validate referral information, including demographics, insurance coverage, and clinical eligibility for home health.
• Collaborate closely with hospital case management, discharge planners and physicians to coordinate safe and timely transitions to home.
• Serve as the primary liaison between the hospital and home health agency to ensure continuity of care.
• Maintain real‑time communication with interdisciplinary teams regarding referral status, delays, or changes in discharge plans.

• Active clinical license in the state of Florida
• Minimum of 2 years of home health experience required
• Strong clinical assessment and critical thinking skills to support patient care planning and coordination.
• Support business development efforts within facilities
• Experience with hospital portals preferred
