The Transition Counselor serves as a trusted advisor and primary case manager for transitioning Soldiers and eligible clients, providing individualized counseling, group instruction, and comprehensive transition support throughout the military-to-civilian transition process. This position plays a critical role in helping clients successfully prepare for post-military employment, education, entrepreneurship, vocational training, and other career pathways. /p pThe successful candidate will guide participants from initial counseling through Capstone validation, helping them develop and execute Individual Transition Plans (ITPs), complete Career Readiness Standards (CRS), and access available resources and support services. The Transition Counselor must be skilled in case management, adult learning, counseling, customer service, and stakeholder coordination while ensuring compliance with Army TAP policies and contractual requirements. /p h3Summary /h3 pThe Transition Counselor provides individualized counseling, group instruction, and lifecycle case management to transitioning Soldiers and eligible clients. This position serves as the primary case manager from enrollment through transition completion, helping participants achieve transition readiness, meet Career Readiness Standards, and access resources necessary for long-term success. /p h3Key Responsibilities /h3 ul liConduct initial counseling sessions, assess transition readiness, interpret assessment results, and assign appropriate service tiers in accordance with Army TAP requirements. /li liAssist clients in developing, updating, and executing Individual Transition Plans aligned with employment, education, entrepreneurship, vocational training, and personal goals. /li liDeliver individual counseling, classroom instruction, workshops, briefings, and virtual services using Government-approved curriculum and instructional materials. /li liMaintain continuous case management engagement and required follow-up throughout the transition process, from enrollment through Capstone completion. /li liGuide clients in completing Career Readiness Standards and required documentation, including DD Form 2648 and associated records. /li liSchedule and reschedule appointments, conduct client follow-up activities, identify participation barriers, and coordinate appropriate interventions. /li liDocument all services, assessments, milestones, referrals, warm handovers, and follow-on actions accurately and within established timelines. /li liCoordinate referrals and warm handovers to the Department of Veterans Affairs, Department of Labor, educational institutions, employment resources, community organizations, and other support agencies. /li liSupport intake, registration, classroom logistics, attendance tracking, records management, participant surveys, and other administrative functions. /li liParticipate in quality assurance reviews, staff training, reporting activities, outreach initiatives, mobile support efforts, and surge coverage assignments as required. /li /ul h3Required Skills /h3 ul liStrong counseling, coaching, and interpersonal communication skills. /li liAbility to establish rapport with diverse populations and support clients with varying transition readiness levels. /li liKnowledge of case management principles and customer service best practices. /li liStrong written communication and documentation skills. /li liAbility to facilitate classroom instruction and deliver effective presentations in both in-person and virtual environments. /li liStrong organizational and time management skills. /li liAbility to manage multiple client cases while meeting deadlines and compliance requirements. /li liProficiency with Microsoft Office Suite, including Word, Excel, PowerPoint, Outlook, and Teams. /li liAbility to protect personally identifiable information and maintain confidentiality. /li liAbility to obtain and maintain required Government suitability, installation access, Common Access Card (CAC), and system access. /li /ul h3Preferred Skills /h3 ul liKnowledge of Army Transition Assistance Program policies and procedures. /li liFamiliarity with Career Readiness Standards, Individual Transition Plans, and Capstone processes. /li liKnowledge of military transition challenges and available veteran support resources. /li liAbility to coordinate services across multiple agencies and community partners. /li liExperience providing virtual counseling and instruction. /li /ul h3Required Experience /h3 ul liBachelor's degree from an accredited college or university. /li liMinimum of two (2) years of experience counseling or advising individuals regarding employment, education, career planning, benefits, military transition, workforce development, or community resources. /li liExperience supporting the Army Transition Assistance Program or similar workforce development and career readiness programs. /li liExperience providing case management services and maintaining detailed client records. /li liExperience facilitating group instruction, workshops, presentations, or training programs. /li liExperience utilizing computer-based systems to document services and manage client information. /li /ul h3Preferred Experience /h3 ul liPrior military service, military spouse experience, or direct experience supporting Service members, veterans and military families. /li liExperience supporting the Army Transition Assistance Program or similar workforce development and career readiness programs. /li liKnowledge of Army TAP curriculum, Career Readiness Standards, Capstone requirements, SkillBridge/Career Skills Programs, and interagency transition resources. /li liProfessional certification in career counseling, workforce development, coaching, human resources, or a related field. /li liExperience providing services in military, veteran, government, workforce development, or higher education environments. /li /ul pCALIBRE and its subsidiaries are an Equal Opportunity Employer and supports transitioning service members, veterans and individuals with disabilities.
